Pre-memory clinic support for patients

DCAN (Dementia Change Action Network) have published the Next Steps website to support people who are waiting for a memory assessment appointment. The website is for anyone who has been referred for an appointment at a memory assessment service as well as family members, carers, and organisations who support them. It helps people to find the right support at the right time and includes information on what to expect from the assessment process and ways to support wellbeing. 

Dementia Friends

Become a Dementia Friend - A Dementia Friend learns a little bit more about what it's like to live with dementia and then turns that understanding into action - anyone of any age can be a Dementia Friend. Whether you attend a face-to-face Dementia Friends Information Session or watch our online video, Dementia Friends is about learning more about dementia and the small ways you can help. From telling friends about Dementia Friends to visiting someone you know living with dementia, every action counts.

If you are interested in becoming a Dementia Friend, please leave your details with Reception and we will contact you when we are able to organise an information session with our Dementia Champion.

Dementia Support Service (DSS)

This service provides advice & guidance to people with mild cognitive impairment (MCI) and dementia and is provided by LPFT. This service includes information provision, care navigation, signposting and post-diagnostic support. The service accepts referrals from the public, patients and health & care professionals.
